Supply of medical devices for cryoablation procedures together with rental of a generator and an electrophysiology system

Summary

The tender concerns the supply of medical devices for cryoablation procedures together with the rental of a generator and an electrophysiology system. The contract runs for 24 months, with an optional extension of up to 6 additional months. The bidder must submit product evidence showing that the offered items are authorised for sale and comply with Regulation (EU) 2017/745, and provide catalogues, leaflets, labels or instructions from the manufacturer or authorised representative for items 1–7. The notice also refers to exclusion grounds under Article 108(1) and (2), the Polish sanctions law related to aggression against Ukraine, and Article 5k of Regulation (EU) No 833/2014.
