Summary

The tender concerns the supply of various medicines. The contract is divided into 20 lots/tasks. The detailed item list is in the price schedules attached to the tender documents. The award criterion is price only. The bidder must submit the European Single Procurement Document, and the highest-ranked bidder must later provide a criminal record certificate and a declaration on group membership/non-membership. Foreign bidders must provide an equivalent register extract or document from their home country. The bidder must also confirm that the medicines are authorised for sale and use in Poland, that transport meets Good Distribution Practice requirements, and that the technical forms for lots 1.2 to 1.21 are completed.
