Summary

The tender concerns the supply of surgical sutures, staplers, clips, implantable meshes and haemostatic products as medical devices. The contract is divided into 25 lots/packages. The bidder must submit the European Single Procurement Document (ESPD/JEDZ) with the bid and, on request, criminal record evidence, group-affiliation declarations and updated confirmations regarding exclusion grounds. For the offered medical devices, the bidder must provide proof of market and use authorisation, a product description/catalogue, leaflet or technical sheet in Polish, a declaration of conformity, and, where applicable, the device class and UDI-DI code. Missing catalogue or UDI-DI data may lead to rejection of the bid.
