The bidder must confirm the required declarations and, where applicable, provide details of convictions, self-cleaning measures and electronic evidence. The bidder must submit the completed pricing form and confirm that the bid complies with the requirements set out in the procurement documents. The bid price must be entered according to the structure of the pricing form. The bidder must have at least one medical technology specialist in its team who has the training and at least 3 years of experience in maintaining and repairing the equipment groups listed in the lot, or who has organised the maintenance and repair of such equipment together with the relevant manufacturer. Supporting documents must be provided for this requirement. In addition, the bidder must submit a manufacturer’s confirmation showing that spare parts, maintenance consumables and the necessary training are available; if the letter is not in Estonian or English, an Estonian translation must be attached. The ESPD also contains general confirmations that the bidder can provide proof of payment of taxes and social security contributions or allow the contracting authority to verify this information electronically.
