According to the compliance requirements, the bidder must submit the completed bid form, technical information material, a document proving the manufacturer’s or authorised supplier’s right to represent the offered products, and the packaging label for each offered product. For medical devices, the instructions for use, the manufacturer’s declaration of conformity and, where applicable, the EC certificate of conformity must also be provided. The bid price must be submitted using the prescribed pricing form.
