Compliance requirements
The bidder must confirm that neither the bidder nor its management, representation or supervisory body members have been finally convicted in the last five years for participation in a criminal organisation, corruption, fraud or any other exclusion ground listed in the ESPD, or that the exclusion period no longer applies. If any exclusion ground exists, the bidder may submit evidence of self-cleaning measures to restore reliability. Where relevant, the ESPD requires the date of conviction, the reason, the person convicted, the duration of exclusion and electronically available evidence.
Qualification criteria and exclusion grounds
The bidder must submit an application for participation in the procurement according to Form 1. If the person signing the bid does not have representation rights in the register, a power of attorney on Form 2 must be attached. The bid must include technical descriptions, manufacturer information materials and other evidence showing that the offered product complies with the requirements of Annex 1, including CE marking or other proof of compliance with standards. A document from the manufacturer or an authorised supplier proving sales authorisation or technical support must also be submitted. The bidder’s maintenance team must include at least one medical technology specialist who has completed manufacturer training, and the name of that person together with a certificate or the manufacturer’s confirmation of the training must be provided. The price offer must be completed on Form 4. If an equivalent product is offered, equivalence must be explained and supporting evidence attached.
