The bidder must confirm that neither it nor the members of its management bodies have been convicted in the last five years for participation in a criminal organisation, corruption, fraud and the other offences listed in the ESPD. If any exclusion ground applies, the bidder may submit evidence of self-cleaning measures, where permitted. The tender must comply with the requirements set out in the tender documents and the technical specification. The bidder must submit the application for participation in the procurement on Form 1, a power of attorney on Form 2 if needed, and manufacturer documents or information materials proving product compliance. The price and product data must be entered on Form 4. If an equivalent solution is offered, explanations and evidence of equivalence must be provided.
