The bidder must confirm that neither it nor the members of its management, administrative or supervisory bodies have been finally convicted within the last five years for participation in a criminal organisation, corruption, or any other exclusion ground listed in the ESPD. If an exclusion ground applies, the bidder may submit evidence of self-cleaning measures where this is permitted. The tender must comply with the technical specification and the tender form in the procurement documents. The bidder must provide the descriptions, manufacturer information, certificates, declarations of conformity and other evidence needed to assess the compliance of the offered products. In the case of a joint tender, a power of attorney for the joint bidders must be submitted in form 3. The participation application must be submitted in form 1 and the price offer in form 4. If equivalent solutions are offered, the bidder must explain the equivalence and attach supporting evidence.
