The bidder must confirm that neither the bidder nor the members of its management body have been finally convicted within the last five years for participation in a criminal organisation, corruption, fraud, terrorist offences or offences related to terrorist activity, money laundering or terrorist financing. If such a ground applies, the bidder may submit evidence of self-cleaning measures where permitted. The bidder must submit the price form in the structure required on the "Award criteria and assessed indicators" sheet. The bid price must be prepared using the completed Form: Price form, Annex 1. The bid must confirm acceptance of all conditions set out in the procurement documents. Conditional bids are not allowed.
