Compliance requirements
The machine-readable notice included the standard exclusion grounds for criminal organisation, corruption and other conviction-based grounds, together with the usual self-cleaning option where applicable. The bidder must confirm the absence of the listed exclusion grounds and, if relevant, provide the conviction date, reason, person concerned, exclusion period, self-cleaning measures and evidence. The notice also refers to tax and social security payment confirmations. The exact exclusion grounds and any additional confirmations must be checked in the tender documents.
Qualification criteria and exclusion grounds
The machine-readable notice did not include precise qualification, suitability or compliance requirements; these must be checked in the tender documents. As compliance requirements, the bidder must submit the completed participation application form, the completed list of goods and unit prices, and the technical description form. Manufacturer technical documentation or other proof of conformity must be provided for the offered product. For lot 1, compliance with EN 14126, EU Regulation 2016/425 and the manufacturer’s ISO 13485 or ISO 9001:2015 certificate must be proven. Samples are required: at least 2 samples of each offered product in each size, in original packaging. In the case of a joint tender, a power of attorney for the joint bidders must be attached.
